Group education and counselling for individuals with pre-diabetes. Focus is on providing information and tools to help prevent the onset of type 2 diabetes amongst high-risk groups.

Chinese Cancer Support Group

An educational program that offers guidance and mutual support to Chinese cancer patients and their families. The group is led by a Chinese-speaking outreach worker. Health professionals such as doctors, cancer nurses, pharmacists, radiation therapists, dietitians and social workers are invited to make presentations and answer questions.

Toronto Community Hepatitis C Program

The Toronto Community Hepatitis C Program (TCHCP) is a partnership between South Riverdale Community Health Centre, Regent Park Community Health Centre and Sherbourne Health Centre.

Community Information

The COUNTERfit harm reduction program began in response to health and community concerns. It has proven to be one of the most effective programs in Toronto, and a model for other harm reduction efforts. Now urban health and harm reduction staff present workshops and information sessions about COUNTERfit programs and services.
