South Riverdale Community Health Centre (SRCHC), a non-profit, multi-service organization that provides primary healthcare, social and community services, health promotion, and disease prevention hosts regional programs and city wide services in partnership with community service and health providers across the city, such as Choose Health Self-Management Program, DECNET the Diabetes Education Community Network of East Toronto, Diabetes Eye Screening Program and Diabetes Care Connect Referral Service.
Choose Health
Choose Health’s mandate is to enhance patient self-management and provider health literacy using evidence-based trainings and programs. The service is offered in partnership with 33 primary care, health and social service sector organizations across Toronto. Programs are delivered by trained peer-facilitators.

Diabetes Eye Screening Program
The Diabetes Eye Screening Program is hosted by South Riverdale CHC, funded by the Toronto Central LHIN, and in collaboration with Dr. Michael Brent (MD FRCSC) and University Health Network, Ontario Telemedicine Network (OTN) and Clarion Medical Technologies Inc.
DECNET
DECNET, the Diabetes Education Community Network of East Toronto, is a diabetes education program that provides counselling, education and support to help adults manage their type 2 diabetes or prediabetes.
