Program Facilitator – Choose Health

South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Program Facilitator – Choose Health supports various components of the Choose Health Program. This position facilitates logistical support from supporting program facilitators, participant registrations, file management, data entry, program material, supply preparation and maintaining the overall workflow of the program including specific protocols and processes.

This position is expected to work onsite.

Responsibilities:
• Support all Peer facilitated and Health Care Provider Choose Health programming primarily supporting Peer led workshops, e.g. Chronic Disease, Chronic Pain, toolkit, stay connected Fridays- ROM, AGO, TIFF
• Support the delivery of quality, evidence based self-management programs and services for program participants
• Facilitate recruitment of both peer facilitators and community members for community-based programming as needed
• Assist ongoing peer facilitator training and coaching sessions
• Support and mentor a team of facilitators with lived experience in collaboration with the Choose Health – Health Promoter
• Provide coverage for facilitator absences as required
• Monitor Choose Health email inbox and primarily respond to general inquires and community-based programming related emails/requests (facilitators and potential participants) and direct emails to colleagues
• Participate in minute taking and other team functions
• Provide registration supports, e.g. reminder calls for participants and email communication
• Prepare and ship material for community-based workshops and health care provider workshop material as needed
• Support the active monitoring and evaluation of on-going programming and services
• Book Zoom meetings and provide participants with support in joining Zoom based virtual programs
• Provide technical virtual support and coverage as needed
• Book program participants following established protocols
• Support incoming referrals following established protocols
• Organize mass mailings
• Maintain inventory of materials and supplies as required
• Provide administrative support for sign off of contracts
• Routinely compile and enter data as per program directives and organizational requirements
• Order and ensure the availability of supplies, program materials and educational literature across multiple sites
• Organize and process facilitator invoices and other invoices as needed
• Collaborate with program team in the production and promotion of program material, brochures, and flyers
• Support delivery of virtual and in person workshops primarily for community members and health care providers as needed
• Collaborate with Choose Health team and other teams across the organization to identify and provide self-management health education to target groups
• Identify networking opportunities with internal and community partners to ensure referral to available self-management patient education programs and health care provider trainings
• Provide administrative support including photocopying, filing, registration, and reminder calls
• Maintain paper and electronic file management systems following established protocols
• Perform data entry and maintain program database ensuring accuracy and integrity of client and partner information
• Book space, arrange for catering as required
• Assist with virtual and in person workshop set up as needed
• Support community development initiatives in alignment with SRCHC’s vision
• Support the maintenance and update www.selfmanagementtc.ca to promote upcoming chronic disease programs/events
• Organize Choose Health special events, outreach and/or educational opportunities as required
• Participate as a team member in all team functions program planning, team meetings, facilitator meetings, and partner meetings
• Participate in all staff meetings, admin meetings, and other cross-organizational meetings as required
• Maintain and develop professional competence and learning
• Attend relevant training as requested
• All other duties as required

Qualifications:
• Minimum College Diploma
• Clerical experience preferred
• Research knowledge preferred
• Demonstrated respect for diversity, equity, and accessibility with the ability to work collaboratively and positively with others
• Strong oral and written communication skills including effective telephone etiquette
• Excellent organization skills with the ability to multitask in a fast-paced environment
• Strong attention to detail
• Demonstrated ability to work independently and as part of a team in a fast-paced environment
• Awareness of the sensitivity to the health issues of a diverse community and marginalized populations
• Strong proficiency in the use of computer technology and various software applications (advanced knowledge of Microsoft Office – Word, Power Point, Access, Excel)
• Experience with data entry and data analysis
• Excellent interpersonal skills in dealing with the public
• Ability to assist people in a mature, friendly and helpful manner
• Demonstrated ability to be flexible, organized, a self-starter
• Police clearance documentation will be required for employment
• Proof of double COVID-19 vaccination status will be required for employment
